Immunization Requirements
The health and safety of the university community is a priority at UC. We make every effort to ensure that the university is proactive at enhancing the health and well-being of our community. While the immunization requirement alone is unable to stop outbreaks, medical evidence shows that higher levels of immunization among a population leads to greater protection and less severe symptoms should an outbreak occur.
The immunization requirements for UC are based on recommendations from:
- U.S. Centers for Disease Control and Prevention (CDC)
- American College Health Association (ACHA)
- Ohio Department of Health (ODH)
- Cincinnati Health Department
- UC Department of Family and Community Medicine.
All incoming undergraduate, graduate and professional students attending main campus, UC Blue Ash, and UC Clermont must comply with this policy.

Distance Learners
- Immunizations will not be required at this time. (unless enrolled in the College of Allied Health Sciences Medical Laboratory Sciences (MLS) Distance Learning Program)
- UHS advises all students to be up-to-date on vaccines recommended by the Advisory Committee for Immunization Practices [ACIP].
Required Immunizations
Required immunizations for all incoming undergraduate, graduate and professional students attending main campus (East and/or West campus), UC Blue Ash and UC Clermont:
- Hepatitis B – 3 doses or HEPLISAV-B - 2 doses (or titer showing evidence of immunity)
- Measles, Mumps, Rubella – 2 doses (or titer showing evidence of immunity)
- Meningitis A if currently 16 to 21 years of age (one dose given on or after age 16)
- Tetanus, Diphtheria, Pertussis (Tdap) – one booster given within the last 10 years
- Varicella (Chickenpox) – 2 doses, titer showing evidence of immunity, or history of disease
Required Health Screening
Each student must complete a brief electronic tuberculosis (TB) screening questionnaire on the MedProctor site. The questionnaire will appear after registering in MedProctor.Recommended Immunizations

- Hepatitis A
- Human papilloma virus (HPV)
- Influenza - each fall
- Meningitis B
It is the policy of the University of Cincinnati that students show proof of protection against certain communicable diseases. University Health Services has partnered with Med+Proctor to manage the required immunization and TB screening proof.
- Respond to the invitation sent to your official UC Email address from Med+Proctor and register for a Med+Proctor account. You do not have to pay for this process.
- Once you have registered with Med+Proctor, print the official Med+Proctor immunization form.
- Make an appointment with your medical provider or a UHS provider to obtain the required immunizations.
- Upload your competed Med+Proctor immunization form through your Med+Proctor online account. You will be notified by email from Med+Proctor if there are any additional outstanding items related to the immunization requirement.
Students entering Spring 2022 have until February 28, 2022 to upload forms to Med+Proctor.
Students entering Summer 2022 have until June 13, 2022 to upload forms to Med+Proctor.
Students entering Fall 2022 have until October 17, 2022 to upload forms to Med+Proctor.

- Good health for your family, friends, and community members who could be at risk for being exposed to an infectious agent by you.
- Economic benefits from not having to miss class or work.
- Eligibility to enroll in UC classes.
- Students who are not immunized are at risk for developing the communicable disease in the event of an outbreak.
- Students who have not shown proof of immunization may be asked to leave campus and not attend classes in the event of an outbreak (until the outbreak is over).
- You will continue to receive notifications from Med+Proctor until you have completed your immunization requirements.
- The immunization requirement will remain on your Catalyst dashboard until you have completed your immunization requirements.

How do I submit my proof of vaccination?
UC has contracted with an external agency, Med+Proctor, to track and verify students' immunizations. Respond to the invitation sent to your official UC email address from Med+Proctor and register for a Med+Proctor account. You do not have to pay for this process.
I didn't receive an email invite from Med+Proctor. What should I do?
The email invite may take 5 - 7 business days. Don't forget to check your spam mail. If you did not receive one, please contact by email: help@medproctor.com.
I'm not sure if the requirement applies to me, how can I determine if I should submit my immunization documentation?
Review the Vaccination Requirements page, "Who" section. All students required are outlined on this section of the page.
I am an international student. Does this requirement apply to me?
- Yes, all incoming international students are subject to the immunization and health screening requirement.

I will be a new student living off-campus. Will I still need to meet the immunization requirements?
Yes, you will be required to have the same vaccinations as all new students.
I am at UC Blue Ash or UC Clermont. Does this requirement apply to me?
Yes.
I am only taking Distance Learning classes. Does this requirement apply to me?
This requirement does not apply to you if you are in an official Distance Learning program, unless you are enrolled in Medical Laboratory Sciences Distance Learning.
Does this requirement apply to faculty and staff?
This requirement does not apply to faculty and staff unless they enroll as students at the University.
Why are there health screenings for tuberculosis (TB)?
The American College Health Association recommends universal screening of college students (including those without symptoms). Detection and early treatment of an infected individual could prevent potential spread of infection to others.
How do you define “incoming”?
- An incoming student is any degree-seeking student who is newly enrolled in a major/program and attending the University of Cincinnati campus, UC Blue Ash campus or UC Clermont campus. Some examples may include:
- Changing major/programs
- Newly enrolled in a graduate program (even if you were on campus as an undergraduate)
- Transferring to main campus
How/why did you choose these particular vaccines?
- University health officials reviewed national guidelines from the U.S. Centers for Disease Control and Prevention (CDC), the American College Health Association (ACHA), and consulted with medical and public health experts.
- The UC immunization requirement is consistent with those vaccines already required by the State of Ohio for grades K-12 students.
Are there any exemptions/exceptions to this requirement?
A student may submit an application for a Medical or General exemption.
What records do I need to prove my vaccination status?
Download, print, and submit the University of Cincinnati form found on the secure Med+Proctor portal. The form must be filled out and signed by a licensed healthcare provider, and then uploaded back to the secure Med+Proctor portal.
What if I can’t find my records?
- Contact your pediatrician*
- Contact your previous school
- Contact your health department*
- Ask your parents
- Consider having blood tests to verify immunity or receive booster doses. Contact your health care provider or call 513-556-2564, option 2 to schedule an appointment at UHS.
- Check the Immunization Information Systems (IIS) by state provided by the CDC.
*If you were vaccinated in Ohio, you can call your pediatrician office or the health department. Many Ohio records have been transferred to Ohio Impact SIIS (Statewide Immunization Information System). All pediatrician offices and the health department have access to Impact SIIS. Can I get the required immunizations at UC?
Yes, all of the vaccines and tuberculosis testing are available at University Health Services. University Health Services accepts most private insurance, in addition to the campus Student Health Insurance Plan. Call 513-556-3564, option 2 to schedule an appointment.
Are these immunizations covered by insurance?
Since these vaccinations are considered preventive care, they should be covered by your health insurance plan if it is Affordable Care Act (ACA) compliant. Please contact your health insurance plan for questions regarding your specific benefits and coverage.
What happens if I don’t submit proof of immunizations or complete the TB screening/testing?
- Students who are not immunized are at risk for developing the communicable disease in the event of an outbreak.
- Students who have not shown proof of immunization may be asked to leave campus and not attend classes in the event of an outbreak (until the outbreak is over).
- Students will continue to receive notifications from Med+Proctor until they have completed the immunization requirements.
- The immunization requirement will remain on student’s Catalyst dashboard until they have completed the immunization requirements.
- It may affect your ability to enroll in UC classes.
