UC Student Health Insurance (QLE) Enrollment

To enroll in the Student Health Insurance due to a qualifying life event, you must:

• Complete the Coverage Status Change Form - Coverage Status Change Form for QLE (PDF)
• Provide required documentation as listed next to the QLE reason.

These two documents will need to be attached in an email and sent to studins@ucmail.uc.edu for processing.
