Answer the following questions with 500 words or less:

1. What can you contribute (i.e. skills, personality traits) toward a UHP summer internship position?

2. Please describe how you hope to benefit from participating in the UHP summer internship program.

3. Are there any populations or issues that you have a specific interest in working with? Why?

4. Briefly describe any other information you feel would be pertinent to your application.

5. Please list specific time constraints (vacations, weddings, etc.) that your site should know about? (some sites are very strict on the days you need to work).

6. Please list any time restraints on daily start time and end time? (some sites require you stay later than 5pm some days).

7. Are you applying for a full time (40 days) or part time (at least 20 days) internship position? If part time, what days can you work?

Answer the following question only if you would like to be considered for the position of co-director. Please note that UHP co-director work extends through UC II year ($1500) and as a financial advisor during UC III year ($500). Total $5000.

8. Describe any previous experiences that make you a good candidate for the position of UHP co-director (i.e. prior work/project experience, leadership, organizational, public speaking, business, management or communication, or grant writing skills or experience).

We will send your application to each indicated site. if, after you interview, you decide to change your ranking, please let the current co-directors know ASAP. Please remember that not all sites conduct interviews.
