All residents and fellows who wish to rotate at the University Hospital must
complete the Visiting Resident Application including all attachments 4 weeks prior
to the start of the rotation. If the application is received in the Office of GME
after the 4 weeks, the application may not be accepted.
Please note that the application must have the signature of the Resident/Fellow's
Program Director from their Parent (Current/Home) Institution and the signature of the University Hospital's Program Director to which they are rotating before submission to the Office of GME.

Please use this Rotator Checklist and the attached forms to the checklist in order to ensure
that all documentation and paperwork has been completed in full prior to submitting to the

Residents from other Health Alliance Hospitals need to use the Rotator Checklist to ensure they have the appropriate paperwork but do not need to include the Health Alliance Employee Health Release or LastWord Access Form. All other forms need to be completed and returned 4 weeks prior to rotation.
