If you need to take a Medical Leave of Absence (includes Maternity Leave,
Paternity Leave), you need to contact Health Alliance Employee Health
directly at 585-6600. When taking a Medical Leave of Absence, you may
be eligible for Short Term Disability (STD). If you have questions regarding
your benefits, please contact Benefits direclty. You do not need to complete these
forms if you are taking Funeral Leave due to a death in the family. See Benefits.

Duty Hours
The University Hospital is committed to making the work life of a resident
and their family an enjoyable time. With the new ACGME Duty Hour Regulations
in place as of July 1, 2003, the Office of GME has created a Duty Hours
Hotline 513-584-2109. This is an anonomyous hotline to report any duty
hour violation (over the 80 Hour Work Week, not having 1 in 7 off from
clinical duties, more than every third night call, or not having 10 hours in
between clinical duties). We encourage residents, faculty, staff, and family
