When a Health Alliance Associate has a "Work Life/Event"
(i.e. Adoption, Baby, Marriage, Death etc), it is imperative
that the Associate contact Employee Benefits and Employee
Health to inquire about changes and forms that may need to
be completed. One form may be the Benefits Enrollment
Change Form.*
Funeral Leave
Health Alliance Associates recieve up to 3 days of paid funeral leave
for the immediate family within a seven day calendar day. The following
family members qualify as immediate family: your parent, foser parent,
parent-in-law, *parent of domestic partner, sister, brother, child of
associate, child of spouse or *domestic partner, grandparent or grandchild.
(*Domestic partner benefit only applices if the associate completed a
Declaration of Domestic Partnership prior to the death of the domestic
partner or the domestic partner's family member.)
