UDC Appointments
Making an Appointment

To schedule an appointment with our physicians, please call (513) 475-7630 between 8:00 a.m. and 5:00 p.m. or call one of our satellite offices. Patients are seen by appointment as this enables our staff to function effectively and minimizes your waiting time.

Emergencies/Urgent Conditions
During office hours (8:00 a.m.-5:00 p.m.), please call (513) 475-7630. When our office is closed, please call (513) 584-1000, and ask for the dermatology resident on-call.

New Patients Are Welcome
Please arrive early for your first appointment to complete any necessary forms. If you belong to a managed care plan that requires a referral from your primary care doctor, please bring this referral form with you or contact our office so that we may assist you in obtaining one. We will need your insurance information at the time of your visit. Please bring your insurance card with you.


FEES, COPAYMENTS, AND INSURANCE
University Dermatology Consultants' physicians and staff are concerned about the cost of medical care. We wish to provide each patient exceptional value, service, and convenience. To hold down the cost of your care, we continually review the efficiency of our office and billing operations. Please inform your doctor if you are experiencing financial difficulties and we will try to assist you.
Fees and Copayments
To help us maintain our fees at the lowest possible level, we ask that payment of fees be made at the time of your visit. All copayments for HMO and PPO patients are required at the time of service. For your convenience, we accept cash, personal checks, VISA, and MasterCard. Balances due from deductibles or excluded services are due within 30 days of receipt of a statement from University Dermatology Consultants, Inc. Unattended accounts are referred to a collection agency after 90 days.
Waivers
Some services may be determined by the insurer to be unnecessary under the terms of its policies. In accordance with insurance requirements, we ask patients to sign a written agreement of financial responsibility. Payments for Cosmetic Services Payment for cosmetic service is required when registering at the front desk. We do not file claims for cosmetic procedures with insurance carriers.
Insurance
As a courtesy, we will file appropriate claims with your insurance carrier. Any balance remaining on your account, after insurance or Medicare payment and adjustments are made, is your responsibility. You may have questions concerning your particular insurance coverage. These questions should be discussed with your insurance company because policies vary widely. If you need assistance in this process, we will do our best to help you.
